The undersigned, as bidder, declares that the only person or parties interested in this proposal
as principals are those named herein, that this proposal is made without collusion with any
other person, firm or corporation; that he has carefully examined the Notice to Bidders, the
Instruction to Bidders, this Bid liorrn of Agreement, the General Notes and Specifications,
and the Plans therein referred to, and has carefidly examined the location, conditions and
classes of materials of the proposed work; and agrees that he will provide all necessary labor,
machinery, tools, apparatus, and other items incidental to construction, and will do all the
work and furnish all the materials called for in the Contract and Specifications in the manner
prescribed therein and according to the requirements of the City as therein set forth.
It is understood that a General Liability insurance policy with the City of Friendswood
named, as Additional Insured will be provided.
It is understood that the following quantities of work are to be done at lunip sun price and
are intended principally to serve as a guide in evaluating bids. It is further understood that all
items of work for which there are no specific pay items are included in the bid items sho'�Nrn

It is further agreed that the quantities of work to be done and materials to be furnished are
necessary in the opinion of the City, to complete the work fully as planned and contemplated,
and that all quantities of work, whether increased or decreased are to be performed at the
lump sunk price set forth below except as may be provided for elsewhere in these documents,
The purpose of the project is to purchase and install one new playground structure at
Stevenson Park.

ADDENDUM TO BID
1. Prices submitted include equipment, delivery, installation and wood mulch safety surfacing. If
unitary rubber surfacing is desired, add $18,478 (Eighteen Thousand Four Hundred Seventy-eight
and no/100 dollars) to the base bid. Tax has not been included.
2. Please note that Landscape Structures Inc. playstructures as submitted, conform to accessibility
requirements of the American Disabilities Act as well as ASTM and CPSC guidelines. See
attached Landscape Structures Ina. Warranty Certificate and Product Liability Insurance
Certificate.
3. This quote has been prepared with dimensions and site conditions as given to us.
4. Standard Terms and Conditions For Installation — Playground Equipment and Site Furnishings
Please include these conditions for all purchase orders that include installation.
The Owner is responsible for:
a. Security of equipment while on site.
b. Water, power and trash container on site. Free and clear vehicle access to site.
c. Site preparation, leveled to grade (free of all sand and debris) and ready for installation of the
equipment. For surface mount installations on concrete slabs, a 1 % slope is maxirnunn. (1 /8"
per foot)
d. All utilities located and marked, Movement of any utilities conflicting with the installation of
the equipment is the responsibility of the owner.
e. Once the installers are on site, any delays or additional work incurred by the installation crew
and caused by others (owners or other contractors) will be charged at a cost plus basis.
f. Any additional costs, i.e, demolition, relocation, excavation, surfacing materials, labor due to
abnormal soil conditions, are not included in price.
g. Notify Landscape Structures Inc. of any changes immediately, If changes are made that we
are not aware of and the changes alter the installation, all additional costs will be passed on to
the owner.
Installation quote is for basic installation and does not include core drilling, saw cutting, soil
removal, or hard rock drilling (more than ten nninutes per 12" x 24" hole).
Permits
Any Federal, State and local permits required for this work are the responsibility of the
contractor/owner,
Installation
Landscape Structures hie. is obligated to provide the equipment and the installation of the
equipment by a certified Landscape Structures Inc. installer on a prepared site.
Surfacing is to be provided by others following installation of playground equipment and is
required per ASTM 1487-01, 11.2.2. Installation quote is based oat ground and weather
conditions that permit installation per manufacturer's guidelines,
5. Order will not be considered complete without color specifications, correct addresses for billing,
shipping, and maintenance information. Once the equipment is ordered, an estimated shipping
date will be provided.
6. All structures have been bid with Surface Mount installation assumed. If Direct Bury installation
is required on any of the structures, unit pricing and equipment would change from base bid.
7. Terms: Net 30 Upon Landscape Structures Inc. Credit Approval.
8. Delivery; 3-4 weeks After Receipt of Order with installation to follow delivery of equipment.

Concept for Proposed Future Phases
Included in this section are layouts and 3-D Color Renderings of proposed
future phases for Stevenson Park. We have planned the location of the
equipment included in this bid so that it can be added to as funds are raised.
As future phases are added, more ramps, decks and play components will be
added to eventually reach the 64" deck which is included in this bid. This
future planning will allow the completion of a premier playground structure
that has been designed to be a fully accessible playground for children of all
abilities to enjoy,

All the warranties commence on date of Seller's invoice. Should any failure to
conform to the above express warranties appear within the applicable warranty
period, Seller shall, upon being notified in writing promptly after discovery of the
defect and within the applicable warranty period, correct such nonconformity either
by repairing any defective part or parts, or by making available a replacement part
within 60 days of written notification. Seller shall deliver the repaired or
replacement part or parts to the site free of charge, but will not be responsible for
providing labor or the cost of labor for the removal of the defective part or parts and
the installation of any replacement part or parts. Replacement parts will be

Landscape Structures Inc. has been actively involved with the ASTM (American Society for Testing and
Materials) in the development and on -going updates to the F1487 Safety Standard (Consumer Safety
Performance Specification for Playground Equipment for Public Use) for the design, manufacture,
installation and maintenance of public playground equipment. We are also a charter member of the
International Play Equipment Manufactures Association (IPEMA).
All products covered under the scope of the Standard in our 2007 Playground Equipment Catalog, have
been tested and certified to be in compliance with the requirements of the ASTM F1487 Safety Standard,
except where noted. Our facilities, procedures and test results have been validated by an independent
testing laboratory according to procedures set forth by the IPEMA, To verify compliance of all our play
components, visit the IPEMA website at www.ipema.org.
To the best of our knowledge, we also conform to the U.S. Consumer Products Safety Commission's
(CPSC) Handbook for Public Playground Safety published in 1998, Our playstructures are also designed to
be in compliance with the ADA Accessibility Guidelines.
The company achieved ISO 9001:2000 certification in March 2003 and the ISO 14001 environmental
standard in Sept 1998. The pursuit of ISO certification helped Landscape Structures establish its quality
management systems and establish the infrastructure for continued growth.

The Revolutionary Idea of Continuous Play
Landscape Structures was incorporated in 1971 by Steve and
Barb King. Steve was a landscape architect with a keen eye for
beautiful aesthetics and functionality, and a particular interest
in applying these skills to the playground environment. Barb
was a home economist with a talent for organization, and the
inherent ability to plan, coordinate and execute multiple tasks.
Their contrasting styles and backgrounds would prove to be a
tremendous asset as they strove to turn an idea into a
successful business.
Landscape Structures' initial playground products were
constructed in Barb and Steve's garage, and, like most
playground equipment, were made almost entirely out of
wood However, Steve's play equipment designs were unique
in one important respect: The play components - slides,
horizontal ladders and climbers - were interconnected and
linked together. Steve termed this concept "continuous play"
and reasoned that this allowed him to create more play value
in a smaller space, and encourage more social interaction and
skill development among children. This concept proved
revolutionary in the playground industry and established the
foundation for the growth of the company.
Persistence Brings Growth
In the beginning Barb and Steve did it all. They called on
public parks, schools, child care centers and housing
developments, and presented their designs and philosophy.
Once hired, they bought the material, fabricated and
constructed the playstructure, and installed it themselves.
They also managed all the day-to-day business functions of
accounting, insurance and banking. For years this pace kept
them working 18-hour days and living from project to project.
In the mid 1970s, Barb and Steve made the big step of hiring
independent sales representatives to sell their products
throughout the U.S. As sales grew, so did the demands on
manufacturing and Landscape Structures began renting a
small warehouse/manufacturing facility in Golden Valley, In
1975 the company built a larger facility in Independence, Minn.
Within a few years, the company also outgrew this facility, and
in the early 'BOs Barb and Steve King constructed a new
manufacturing and warehousing building in Delano, Minn., a
location that still serves as the company headquarters. In 1984
the company purchased Mexico Forge, a manufacturer of steel
and aluminum playstructures, and began moving out of wood
construction entirely. Once again, Steve saw an opportunity to
provide school and park playgrounds with greater safety,
durability and fun using the new materials and he never looked
back. Today Landscape Structures employs nearly 300 people
and has multiple manufacturing facilities totaling 340,000
square feet.
